This contact us form is not functional on the demo site. But we have included the instructions to show you what's available. You can add as many fields as you wish. The script will only allow the email to be sent to the address you desire. This form cannot be used for spam.

NOTE: If you do not delete these instructions, the world will be able to see these instructions. We recommend you delete them as soon as possible.

1.Fill out the email address you want your form to be sent to.
Double click on the text box that has "enter email" and enter the email address
the form will be emailed to. Make Sure you add this email address to the list of ALLOWED EMAIL ADDRESSES in your Admin Panel. Use the envelope icon at the bottom right of the admin panel to do this.

2.Choose your Required Fields.
At the moment firstname,lastname,email & comments are required. If the user doesn't fill out these fields they will be told to go back and fill them out. To customize your list just add more fields to the required fields text box. The following is the entire list of fields: firstname,lastname,address1,city,state,zip,phone,email,comments The second text box starts with FirstName. These correspond to the fields in your form. If a visitor doesn't fill out one of these fields, the form will let them know they missed a required field. The field names need to be comma separated.
3.The REMOTE_ADDR & REMOTE_ADDR, HTTP_USER_AGENT is for technical checking.
The REMOTE_ADDR will include the IP Address of the visitor and the HTTP_USER_AGENT will include the browser type (Internet Explorer, Netscape ect...). There is no need to edit this text box. Leave it or Delete it.
4.Enter your Email Subject.
The text box that says "enter an email Subject" will be the subject heading for your email inbox. We recommend that you create a descriptive subject heading so you know the email is coming from your website. That way you don't accidently thinks it's SPAM and delete it.

5. DELETE THESE INSTRUCTIONS.
If you don't delete these instructions, they will be posted on your website.
